State DCCA Announces Disciplinary Actions Against Licensed Professionals Through June 2025

Honolulu, Hawaiʻi – The Hawaiʻi Department of Commerce and Consumer Affairs (DCCA), along with its various state Boards and Commissions, has released a summary of disciplinary actions taken through June 2025. These actions involve individuals and entities holding professional and vocational licenses across Hawaiʻi, with dispositions resulting from either contested case hearings or settlement agreements.

Settlement agreements allow respondents to resolve claims and avoid the expenses of administrative hearings, though they are subject to Board approval. The DCCA and its oversight bodies are tasked with ensuring licensed professionals adhere to state law standards.

Board of Barbering and Cosmetology:

Madali LLC dba King Cuts: Fined $500 for allegedly permitting an unlicensed person to perform barber activities. (Effective 6-3-25)

N&J Nails LLC and Thanh Ngan Ngyuen: Fined $1,000 for allegedly permitting an unlicensed person to perform beauty operator activities. (Effective 6-3-25)

Kiani K. Costabrum aka Kiana K. Costabrum (Kaua’i) dba Bare Beauty Kauai: Fined $2,000 for allegedly offering and providing facials and/or lash extension services at a location not registered as a beauty shop. (Effective 6-3-25)

Mikey’s Barber & Hairstyling Salon LLC and Canh T. Nguyen: Fined $500 for allegedly permitting an unlicensed person to perform barbering activities. (Effective 6-3-25)

Board of Nursing:

Christine D. Caguioa: Received a 2-year license probation, continuing education requirements, and a $750 fine. Allegations involved using another nurse’s login credentials to enter patient weight data without authorization. (Effective 6-6-25)

Real Estate Commission:

ALEXA RAE THROPP (Hawaiʻi Island): Fined $750. Allegations involved a plea of nolo contendere on May 16, 2024, to one count of Operating a Vehicle Under the Influence of an Intoxicant. (Effective 6-27-25)

Myriam Haynal (Hawaiʻi Island): Fined $7,000. Allegations involved a conviction on May 13, 2024, for Driving Under the Influence of Alcohol. (Effective 6-27-25)

Board of Pharmacy:

RPC2B, LLC: Fined $900. Allegations involved being disciplined by the state of New Jersey. (Effective 6-19-25)

Walgreens.com, Inc. dba Walgreens #02445: Fined $11,000. Allegations involved discipline by the state of Nevada (August 2024, related to incorrect prescription verification), discipline by the state of Texas (April 2019), failing to timely notify the Board of the April 2019 action, and a false answer on a December 2016 application regarding violations of drug laws. Further discipline by Texas occurred on February 4, 2025. (Effective 6-19-25)

Empower Clinic Services LLC dba Empower Pharmacy: Fined $500. Allegations involved being disciplined by the states of Virginia and Illinois. (Effective 6-19-25)
